Payment, Insurance, and Sliding Scale Fees
At Modern Wellness Collective, we are committed to providing accessible and affordable mental health care. Below, you’ll find details on our payment options, accepted insurance, and sliding scale availability.
We accept the following forms of payment:

✔ HSA/FSA Cards
✔ Secure online payments through our client portal
Payment is due at the time of service unless prior arrangements have been made.
We currently accept the following insurance providers:

📌 Multiple Employee Assistance Programs (EAPs)
If we are not in-network with your insurance provider, we can provide a Superbill upon request, which you may submit to your insurance for possible reimbursement. We encourage you to check with your insurance provider regarding your mental health benefits and coverage.
Sliding Scale & Reduced Fee Options

Cancellation & No-Show Policy
We kindly request 24-hour notice for cancellations. Missed appointments or late cancellations may result in a fee.
